What the inspector wrote

On February 18, 2026, Licensing program analyst (LPA) Naves conducted an unannounced case management visit due to an incident self-reported by the center that occurred on February 11,2026. LPA was greeted by director Diana Gonzalez the purpose of the visit was explained and was granted entry to the facility. 33 staff including Director supervised 57 children on this day. On February 12,2026 it was reported that on February 11,2026 staff took 5 children to the bathroom and thought she had 4. Staff left with the group of 4 and it wasn’t until second group entered bathroom that child was discovered to still be in the bathroom unattended. First staff member did not communicate with other staff members that child was still in bathroom. LPA and Director visited location where incident occurred. A short time Alisa Folda Education and Inclusion Manager arrived. LPA Director and Ms. Folda discussed additional measures to ensure accountability for staff and their group of children being accounted for once returning indoors from activities requiring leaving the classroom. See 809D for deficiencies cited today. A notice of site visit was given and must remain posted for 30 days. Appeal rights given. Exit interview conducted and report was reviewed with the Director Diana Gonzalez.

    No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement was not met as evidenced by: Based on observations file review,and interviews, the center did not comply with the section cited above. The center did not provide sufficient supervision; child was left in bathroom inside stall unattended while staff left with 4 children rather than the 5 she had taken.Child was discovered by other child entering with second group to use bathroom. The center failed to provide adequate supervision to meet the needs of children in care poses a potential health and safety risk for children in care.
