On March 12, 2026, at approximately 10:20AM, Licensing Program Analysts (LPAs) Nicole Tran and Jaclyn Naves conducted an unannounced case management visit at the facility regarding a self-reported incident. LPAs met with Site Supervisor, Stefanie Eaves, and explained the purpose of the visit. Present during the visit was 6 staff and 34 school age children. The facility reported an alleged incident on February 27, 2026. During the visit, LPAs conducted site observations and interviews. Based on interviews and review of pertinent information, there is no finding at this time. The facility complied with reporting requirements and has taken appropriate action to investigate and address the alleged incident. No deficiencies were cited during today\342\200\231s visit. Appeal rights were provided. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Site Supervisor, Stefanie Eaves.
