What the inspector wrote

On May 21, 2026 at approximately 1:35PM, Licensing Program Analyst (LPA) Nicole Tran conducted an unannounced, annual inspection and met with Licensee, Kara Hanks, and Assistant Site Director, Sarah Durkin, and disclosed the purpose of the visit. Present during the inspection was 7 staff and 47 school-age children. The facility operates on the campus of Olive Elementary School. Hours of operation are Monday to Friday from 7:00AM to 6:00PM in Portables 1, 2, and 3. LPA inspected Portables 1, 2, and 3 for health and safety hazards. LPA observed a variety of age-appropriate toys that are in good condition. Classrooms have board games, art supplies, and manipulatives to encourage free play and imagination. There were also spaces for other activities, eating, and rest and relaxation. Age-appropriate furniture was observed to be maintained in good condition, free of sharp, loose or pointed parts. Storage for children's belongings are located inside the classroom. Facility is equipped with multiple fully charged fire extinguishers, first aid kits, carbon monoxide detectors, and a fire alarm system. There are fire sprinklers and a smoke detector in Portable 3. Chemicals, poisons, and cleaning solutions have been made inaccessible to children. For children’s use, there is one individual restroom in Portable 1 and two individual restrooms in Portable 3. Restrooms for children's use were observed to be in working condition. LPA observed children's restroom to have age-appropriate toileting equipment and hand-washing materials. Continued on Page Two Page Two The facility provides afternoon snacks. Food menus are posted inside by classroom doors. Drinking water is from tap and children can refill their water bottles as needed. Drinking water fountains are also available in the outdoor play area. Per staff, there are zero currently enrolled children that require prescription medication. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018 , requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16 . For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 ( PIN 21-21.1-CCP .). LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP . LPA referred facility representative to the Department website for lead: Lead Toxicity Prevention and Water Testing Information ( https://www.cdss.ca.gov/inforesources/child-care-licensing/water-testing-information ). CCC COMPLETED TESTING AND NO LEAD EXCEEDANCES LPA inspected the outdoor area for health and safety hazards. The facility utilizes the Upper Playground of Olive Elementary School. Outdoor area is fully enclosed and fully fenced. Swings and play structures either have rubber flooring or sand underneath to cushion falls. Trees and structures provide shade for children to rest under as well. LPA did not observe any pools, spas or bodies of water on site. Licensing documents are properly posted and available for review in Portable 2. LPA advised Assistant Site Director to post licensing documents in a prominent area in all portables. The facility conducts emergency disaster drills that are documented and maintained. The last disaster drill conducted was on April 30, 2026. The facility utilizes an electronic sign in/out record via an electronic application. Continued on Page Three Page Three LPA reviewed 6 random children's records. Children's files were observed to be complete and included their emergency information and consent for emergency medical authorization. LPA reviewed 5 staff files. There is at least one staff member with current pediatric CPR/First Aid training. Staff’s immunization records were unavailable for review. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Facility representative was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Two Type “B” deficiencies are being cited regarding staff’s records, see LIC 809-D for more information. A plan of correction was discussed and agreed upon with Assistant Site Director. Appeal rights were provided and explained. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with facility representative, Sarah Durkin.
