Inspector’s narrative

What the inspector wrote

Page Two Director states she is aware of all items needing to be repaired and have already been making efforts towards them. LPAs observed ceiling panels ready to be installed and Director said she has pending quotes for repairs. Based on LPAs' observations, interviews, and all information gathered, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. The facility is being cited one Type B citation for buildings and grounds not being in good repair. A plan of correction was discussed. Appeal rights were provided during visit. Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with facility representative, Kara Hanks.

    101238 Buildings and Grounds(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors.This requirement has not been met as evidenced by: Based on LPAs' observations, the facility has misaligned ceiling panels, water stains in the ceiling, rubber base rolls peeling in the bathrooms, a faucet sink is degrading, and the fire alarm pull in one portable is inoperable.These items pose a potential health and safety risk to children in care.
