On May 4, 2026, at approximately 2:30PM, Licensing Program Analysts (LPAs) Nicole Tran and Jaclyn Naves conducted an unannounced case management visit at the facility regarding three self-reported incidents. LPAs met with Director, Stefanie Eaves, and explained the purpose of the visit. Present during the visit was Director, 10 staff, and 37 preschool age children. The facility reported two incidents that occurred within the last month. During the visit, LPA conducted site observations and interviews. Based on interviews and review of pertinent information, there is no finding at this time for both incidents. The facility also reported seeing pests on-site but not in any classrooms. Per Director, a pest control company, Hydrex, recently visited and placed preventative measures. The facility complied with reporting requirements and has taken appropriate action to investigate and address all incidents. No deficiencies were cited during today’s visit. Appeal rights were provided. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Stefanie Eaves.
