What the inspector wrote

On May 20, 2026, at approximately 10:15AM, Licensing Program Analyst (LPA) Nicole Tran conducted an unannounced case management visit at the facility regarding a self-reported incident. LPA met with Director, Stefanie Eaves, and explained the purpose of the visit. Present during the visit was Director, 9 staff, and 42 preschool age children. The facility self-reported an incident that occurred on May 5, 2026. A court order was received by the facility specifying pick-up times and authorized representatives for a child. Based on record review and interview, facility staff did not deny pick-up by a parent when it was not their turn to pick up the child. Any changes to the schedule must be mutually agreed upon by the responsible parties. The facility did not verify whether there was a mutual agreement to change pick-up times. A Type “B” deficiency will be cited today due to a child's court order not being adhered to. The facility has created a pick-up schedule calendar for the child's teachers that shows the dates when authorized representatives may pick them up. Director has discussed with the child's parents about what is required for a change in pick-up times. Community Action Marin's process regarding court orders have been provided to LPA. The facility complied with reporting requirements and has taken appropriate action to investigate and address the incident. The deficiency will be cleared today. Appeal rights were provided. A notice of site visit was provided and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director, Stefanie Eaves.
