What the inspector wrote

On November 17, 2025, Licensing Program Analyst (LPA) Naves conducted a case management inspection and met with the Senior Recreation Supervisor, Gabriela Farias (S1). The purpose of the inspection was explained. Present were 30 preschool children and 10 staff members including (S1) Gabriela Farias. The case management was related to a deficiency cited on 9/3/25 for staff, including the Director, for not having current Mandated Reporter Training Certifications. A due date of 9/12/2025 by end of business day was given. LPA made a case management visit on 11/5/25 to follow up on deficiencies. During the visit and upon review of staff files it was found 3 staff members, including the Director still not have current Mandated Reporter Certifications. LPA again gave a future date of 11/12/2025 by end of business day for the 3 remaining staff members to have their certifications emailed to LPA. Director failed to comply with the dates given above. A civil penalty is being assessed for failure to correct the deficiency cited above. Exit interview was conducted with Senior Recreation Supervisor, Gabriela Farias . The report and Notice of Site Visit was provided. Notice of Site Visit will be posted for 30 days.
