Inspector’s narrative

What the inspector wrote

On May 21, 2026, Licensing Program Analyst (LPA) Van arrived at the child care center to conduct an unannounced Plan of Correction (POC) inspection. LPA met with Director Nicole Ramirez, who granted access to the center. At the time of inspection, there were no children present. According to the director, the children will be arriving at 1:00 pm. During the annual inspection on April 14, 2026, LPA observed that two of the three teachers did not have complete immunization records on file, including missing documentation for measles, mumps, and rubella (MMR). LPA also noted that none of the three staff members present had a valid Pediatric First Aid and CPR certification on file or available for review. During today’s visit, LPA verified, through record review, that the director had corrected all previously cited deficiencies. Complete immunization records for all teachers were obtained and maintained in staff files, and LPA confirmed that the required MMR documentation was present. The director also provided verification that all staff obtained current Pediatric First Aid and CPR certifications, which were available for review. Based on today’s inspection, all deficiencies cited on April 14, 2026, have been corrected, and the facility is now in compliance. A Notice of Site Visit was issued and must remain posted for 30 consecutive days. An exit interview was conducted, and the report was reviewed with Director Nicole Ramirez. The director has been provided with a copy of this report.
