What the inspector wrote

(Page 2) Regarding the allegation of child sustained unexplained injury while in care; Based on interviews and evidence collected, LPA was unable to determine if day-care child sustained injury while in care. Parent handbook states that when injuries occur, first aid is applied, and child\342\200\231s authorized representatives are informed. Regarding the allegation of staff did not meet child's diapering needs; Based on interviews with staff and record review, LPA was unable to determine if staff did not meet child\342\200\231s diapering needs. Based on interviews, it was reported that children's diapers are reviewed and changing by staff as needed. Although the allegations may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is Unsubstantiated. LPA conducted exit interview with the Lead Teacher. Report was explained and Notice of Site Visit was posted during inspection.

    101212(c) Reporting Requirement: The licensee shall notify the Department in writing of his/her intent prior to making any structural changes that reduce the total amount of indoor or outdoor activity space. Such structural changes shall include, but not be limited to, room additions. This requirement was not met as evidenced by: Based on observation, interviews and file review, LPA confirmed facility has installed dividing wall in the outdoor play area, without prior department notification. This poses a potential health or safety risk to children in care.

    101238.2(d)(2) Outdoor Activity Space:Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. This requirement was not met as evidenced by: Based on observations, LPA confirmed canopy in outdoor play area not properly anchored. This poses a potential health and safety risk to children in care.
