What the inspector wrote

On October 15, 2025, Licensing Program Analyst (LPA) Garcia made an unannounced case management visit to the child day care facility listed above and met with Assistant Director, Anahita Bakhtiary . Purpose of the visit was explained to the director. The case management visit is from a self reported incident that occurred on 10/1/2025. The facility had 21 children and 8 teachers supervising the children in three rooms during the visit. According to the Regional Manager, Catherine Trabanino, the day of the incident, October 1, 2025, a staff member left the classroom during nap time. The door then accidentally closed and a total of 5 children were left unsupervised in the Panda Classroom. LPA Garcia interviewed the 2 staff members regarding the incident. LPA also observed the Panda Room is inside the Seals room. Therefore, teachers must go through the Seals room to get through the Panda room. Based on staff interviews, the children napping in the Panda classroom were left unsupervised on the day of the incident. See LIC 809D page for deficiency cited during today's visit. A copy of this report was given to the Licensee and a site visit notification must be posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview was conducted and report was reviewed with assistant director, Anahita Bakhtiary.
