What the inspector wrote

On 5/8/2024, at approximately 1:15PM, Licensing Program Analyst (LPA) Jonathan Tse and Licensing Program Manager (LPM) Ali Zebila conducted an unannounced annual visit at the facility. LPA and LPM met with Director, Carrie Moehnke (D1). LPA explained the purpose of the visit. Present during the visit was D1, three staff members, and 18 school-age children. The facility is in compliance with staffing and ratio requirements on this day. The facility\342\200\231s operating hours are from Monday to Friday, 2:30PM to 6:00PM. The facility is open on Wednesdays from 1:45PM to 6:00PM. LPA inspected the facility for any health or safety hazards. The facility is located on the premises of Tamalpais Valley Elementary School and uses Portable Buildings A and B as classrooms. There are temperature and ventilation controls for the comfort and safety of children in care. LPA observed both classrooms to be equipped with fully charged 2A10BC fire extinguishers. There is a fire alarm system running throughout both rooms. There are functional carbon monoxide detectors. Poisons, cleaning detergents, and other chemicals are stored inaccessible to children in care. Electrical outlets are covered or obstructed by furniture when not in use to be inaccessible. LPA observed age-appropriate toys and learning materials to be present in the classrooms. Furniture is age-appropriate and free of rough or sharp edges. There are manipulatives and art supplies available for children. The facility provides snacks for children in care. Medication is stored out of reach and inaccessible. There are allergy lists posted in each classroom. First aid kits were observed to be complete. Cubbies are available for each child. LPA observed childproof locks on the refrigerator. Children bring their own water bottles from home and refill them with water from a non-contaminating fixture. The facility uses an outdoor play area that contains age-appropriate play structures. Play structures are cushioned by resilient padding in the form of woodchips. There is a sandbox present that is checked daily. There is a fence that is at least four feet high. There is shade available from the buildings in the school. There are no pools or other bodies of water present in the facility. Continued on Page Two Page Two LPA reviewed four staff files, six children\342\200\231s files, and facility records. LPA observed there to be two staff members with current First Aid/CPR training. The facility uses Bright Horizon\342\200\231s Mandated Reporter Training, which is approved by the Department. All staff members have current Mandated Reporter Training. Staff qualifications and immunizations are available for review. LPA observed all children\342\200\231s files to be complete. LPA observed required postings to be posted and available for review. LPA advised that PUB269 (California Child Passenger Safety Law) shall be posted. D1 printed and posted PUB269 during the visit. The facility last conducted an emergency drill on 4/10/2024. Emergency drills are properly logged and documented. The facility had tested its fixtures for lead and received results on 11/10/2022 before the deadline of 1/1/2023. There was an exceedance of lead in a sink, and the facility replaced the fixture and retested it with no exceedance in the second test. Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). Continued on Page Three Page Three LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. This facility provides Incidental Medical Services \342\200\223 IMS. LPA reviewed storage of \342\200\234medication and equipment/supplies, and reviewed children\342\200\231s, personnel, and administrative records. For IMS information see PIN 22-02-CCP. No deficiencies were cited during today\342\200\231s visit on 5/8/2024. A notice of site visit was given and must remain posted for 30 days.
