Inspector’s narrative

What the inspector wrote

On August 6, 2025., Licensing Program Analysts (LPAs) Naves and Van conducted an unannounced case management inspection at the facility today. LPAs met with the new site Director, Stefanie Eaves, and the purpose of the inspection was explained, which was to follow up on 2 unusual incident reports occurring on July 25, 2025, and July 31, 2025. Present during the inspection were 24 preschool-age children, accompanied by five teaching staff. The facility was operating within its licensed capacity and within the required ratio today. The first unusual incident involved a staff member noticing two cockroaches in the dishwasher area. The center took immediate action and submitted a work order for pest control to come out. According to the Director, the pest control company visited the center on August 2nd, sprayed, and set up monitor stations. Additionally, pest control returned approximately a month and a half later to check if additional treatment was needed. The second unusual incident involved a 3-year-old child reporting to their teacher that there were feces on their fingertips, which the teacher confirmed. It was unclear whether the child had an accidental bowel movement during naptime or if the child may have used the toilet independently without wiping properly. According to the director, the teacher insisted on checking the child\342\200\231s pants after washing their hands, but the child refused. LPAs reminded the director to ensure that staff members check on the children after bathroom visits and ensure that the children\342\200\231s hands are properly washed. No deficiencies were issued during the inspection. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report and appeal rights were reviewed with Site Director Stefanie Eaves and Education & Inclusion Manager Alisa Folda.
