What the inspector wrote

On 5/23/2025, at approximately 3:30PM, Licensing Program Analysts (LPAs) Tran and Ly conducted a Plan of Correction (POC) Visit and met with Director Marion Rollins. The purpose of the visit was explained. Present during the visit were 7 staff, including the Director, four infants and thirteen preschool-aged children. The POC is regarding two Type B deficiencies cited on 4/18/2025 regarding incomplete staff files and facility staff were certified for CPR/First Aid training that was not Emergency Medical Services Authority (EMSA) approved. Since the citations, the facility has completed the necessary staff paperwork and had all current staff members trained in an EMSA certified Pediatric CPR/First Aid course. LPAs observed all the required documentation. LPAs cleared Type B deficiencies cited on 4/18/2025 and letter of clear deficiency provided to Director, Marion Rollins. A copy of this report and appeal rights were discussed and have been reviewed with the Director whose signature on this form confirms they have read the reports.
