What the inspector wrote

On May 13,2025, Licensing Program Analysts (LPAs) Naves and Van conducted an unannounced inspection of the Plan of Correction (POC). Upon entering the home, LPAs observed the children were having lunch. The licensee and two assistants supervised ten children (six preschoolers and four infants). The licensee operates within her licensee capacity and limitations. During the facility's annual inspection on April 22, 2025, several deficiencies were identified. The licensee failed to have a current mandated reporter certificate and immunization records and maintain records of the infants' sleeping logs. In today's inspection, the LPAs confirmed that the Licensee had done the infant sleep logs with childs complete names and the 15 minute increments checked. Additional deficiencies cited on April 22, 2025,for no immunizations, mandated reporter training certificate, and lic 9052 lic9108 proof of correction will be sent to LPA via email by due date of May 30,2025 . No deficiencies were observed in today's inspection. Exit interview conducted and report was reviewed with the licensee, Karina Duque. A copy of this report was given to the Licensee with the notice of site visit. Notice of site visit must be posted for 30 days.
