What the inspector wrote

Page Two Based on the totality of information obtained, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. A $500 civil penalty was assessed today. See LIC 9099D for a Type A deficiency cited today. LPA Tran informed the Assistant Director, April Jackson, that this report, dated September 3, 2025, documents a Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care. Also, LPA Tran informed the Assistant Director to provide a copy of this licensing report dated September 3, 2025, that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification. A notice of site visit was given and must remain posted for 30 days. An exit inspection was conducted with the Assistant Director, April Jackson, while the Director, Melinda Richard, was on speakerphone . The report and appeal rights were provided to the Assistant Director.

(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1).This requirement is not met, as evidenced by: Based on staff interviews and facility incident reports. The center did not comply with the above regulation. C1 sneaked out from the center front gate without the teacher's knowledge and was subsequently located by another parent in the parking lot. This poses an immediate health and safety risk to children in care.
