On January 21, 2026, at approximately 3:15PM, Licensing Program Analysts (LPAs) Nicole Tran and Jaclyn Naves conducted an unannounced case management visit at the facility. LPAs met with Licensee, Evelyn Canales, and explained the purpose of the visit. Present during the visit was the Licensee, Licensee's spouse, Licensee's adult son, Licensee's minor son, and three school age children. During the visit, LPAs conducted file review and discussed the required documents to be made available to the Department. LPAs provided Licensee with LIC 126 Entrance Checklist - Family Child Care Homes. Based on record review, Licensee's spouse's mandated reporter training and immunizations were not available for review. Licensee's adult son had completed their Live Scan fingerprints in 2024 but did not show as associated to the facility. LPAs reviewed all related documents Licensee provided in regards to the fingerprints. LPAs observed that there were data entry errors and some information was not entered correctly. Technical violations will be issued for mandated reporter training, immunizations, and criminal record clearance. A notice of site visit was provided and must remain posted for 30 days. Appeal rights were provided. Exit interview conducted and report was reviewed with Licensee, Evelyn Canales.
