What the inspector wrote

On July 15, 2025 Licensing Program Analysts (LPAs) Naves and Van conducted an unannounced case management visit for an unusual incident self-reported to the Department on June 27 2025. LPAs met with Director Jenna Herzog during today’s visit. At the time of inspection, 29 children were present and were supervised by 9 staff including director. LPAs interviewed the director and staff regarding an incident that involved a teacher hitting and slamming her hand down on a table and yelled at a child which startled children in care as well as staff present. According to the director the staff member was placed on administrative leave and sent to training. The center conducted their own internal investigation into the incident and later terminated the staff member. Based on interviews and information obtained during today’s visit see 809D for deficiency cited today. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Director Jenna Herzog

Based on the interview and record review, it was found that the Center didn't follow the regulations cited above, as a staff member used inappropriate interaction by slamming their hand on the table, yelling, and instructing the child to “shut up”. This violation poses a potential health and safety risk to the children in care.
