Incident investigation·April 21, 2026

On April 21, 2026 Licensing Program Analyst (LPA) Naves conducted an unannounced case management visit to the facility mentioned above. LPA was greeted by Director Jenna Herzog and was granted access to the facility. Present in the facility today were 10 staff members including Director supervising 38 preschool children. The facility self reported an incident that occurred on April 8 2026. The incident was reported to the facility of inappropriate tone being used. Based on interview with the director and a review of the report submitted as well as documentation reviewed today this is an isolated incident and no additional action is necessary at this time. Additionally, the facility has scheduled a staff retraining session for staff on tone and body language. Exit interview was conducted with Director Jenna Herzog. The report and Notice of Site Visit were provided and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
