Inspector’s narrative

What the inspector wrote

On 9/3/2025, at approximately 2:45PM, Licensing Program Analysts (LPAs) Tran and Van conducted an unannounced case management visit at the facility. LPAs met with Licensee, Livia (L1), and explained the purpose of the visit. Present during the visit was L1, 2 assistants, four infants, and four preschool age children. On August 25, 2025, the facility self-reported an incident where a child choked on a small rock. L1 immediately performed the Heimlich maneuver on the child and the rock was spit out. L1 says the situation lasted only 5-10 seconds and called emergency services to ensure the child was safe. Paramedics arrived and after checking on the child, they reported the child was fine. This incident was reported to the child's parents while they were on their way to the facility. The facility was compliant with reporting requirements. During the visit, LPAs inspected the outdoor play area where the incident was for any health and safety hazards. The play equipment was in good repair and the artificial turf was free of debris and loose articles. There was shade available for children to play and rest under. With L1, LPAs discussed potential ideas to prevent similar incidents from occurring. L1 mentioned that they had discussed with parents about removing their shoes before stepping on the artificial turf to prevent any rocks or other small objects from being tracked in. L1 also states that she purchased a rake for the facility to sweep the artificial turf for anything that they might miss during daily inspections. LPAs advised L1 to have the outdoor area inspected and cleaned every time before bringing children out to play. L1 stated they understood. Continued on Page Two Page Two No deficiencies were issued at this time today, 9/3/2025. A notice of site visit was given to licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days. Appeal rights were provided. Exit interview conducted and report was reviewed with the licensee, Livia De Arruda Sebba.
