What the inspector wrote
Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi met with applicant Amber Jean Smith for the purpose of an announced pre-licensing inspection at 201 Seminary Drive Mill Valley, CA 94941 on 1/3/2025 at approximately 10:15am. Applicant is requesting 15 toddler children from ages 18 to 36 months and 30 preschool children ages 3-6 for a total capacity of 45 children. The program will operate Monday through Friday from 8:00am to 5:00pm.
LPA reviewed with applicant the LIC 311A, Records To Be Maintained At The Facility, for child\342\200\231s records, personnel records, administrative records, and documents to be posted.
INDOOR ACTIVITY SPACE:
There are two preschool classrooms (Blue and Red) one toddler classroom (Yellow) and one recreation room located at the facility, and one staff restroom located downstairs. The isolation area for sick children will be located in the main office and the staff break room will serve as the isolation area for staff. The facility will not be providing food services and instead will be provided by parents, the food preparation area will be used to store meals and snacks only and will be inaccessible to children, and staff was reminded that all storage areas shall be kept clean and free of litter and rubbish, and measures shall be taken to keep all such areas free of rodents and other vermin.
LPA measured the classrooms and recreation room which includes a total of 3,849.53 square feet and will accommodate the applicants request for 15 toddler children and 30 preschool children. LPA observed a sufficient amount of equipment, toys, tables, chairs, napping mats, and cubbies in each room. There are first aid kits located in each classroom and the main office as well.

Medications will be stored and inaccessible to children in locked storage cabinets and accordingly to each individual child in each classroom. Cleaning and disinfectant agents will be stored in the storage room located next to the main office. Drinking water will be provided for children via filtered drinking water and parents will provide drinking water as well. LPA observed a functional carbon monoxide detector in each classroom. A paper sign in and sign out process will be used. Parents will be responsible for providing blankets, and pillows for their children and the facility will provide sheets. There is a total of 3 toilets and 3 sinks available for children, and one changing table within arm's reach of a sink with a changing pad that is at least one inch thick with raised sides that are at least three inches tall in the toddler classroom. There is a dedicated landline for the facility and each classroom will have a telephone line as well. Staff was informed facility shall be kept clean, safe, sanitary and in good conditions at all times to ensure the safety and well-being of children, employees and visitors.
OUTDOOR ACTIVITY SPACE:
There are two outdoor activity areas located in the facility, one for toddler children and one for preschool children. LPA measured the toddler outdoor play area and includes a total of 2,241.46 square feet and will accommodate the applicants request for 15 toddler children. The preschool outdoor play area contains a total of 3,948.34 square feet and will accommodate the applicants request for 30 preschool children. The play structure located in the preschool outdoor play area is appropriate for ages 2.5 and up as per Applicant Amber Jean Smith. LPA advised applicant to ensure age ratings for play structures are being adhered to. There is sufficient resilient padding located in both outdoor play areas and there are no bodies of water located on site. There are sufficient toys, play equipment, and materials located in the outdoor play areas. There is sufficient shade provided by umbrellas and surrounding trees in both outdoor play areas. Applicant stated during inclement weather, the recreation room will be utilized with planned indoor activities according to each age component. Staff was advised to inspect the outdoor play areas, sandboxes and play structures daily for any potential hazards to children before and after use.

LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to department within 10 working days.
Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Assembly Bill (AB) 2370, /chapter 676, Statues of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-year after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP)
