What the inspector wrote

Centralized Application Bureau (CAB) Licensing Program Analyst (LPA) Jamel Maiwandi met with applicant Angela Evans for the purpose of an announced pre-licensing inspection at 140 Drake Avenue, Marin City at approximately 10:40am. Also present was Administrative Assistant Shayna Lee, Director Mona Chivers, and Exective Director Stevie Lee . Applicant is requesting 40 preschool children ages 3-5 and 20 toddlers ages 18-36 months. Facility will operate Monday through Friday from 8:00am to 4:30pm. Fire clearance was granted on 12/3/2024. Applicant acknowledges that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, Effects of Lead Exposure, car seat poster, menus, and daily schedule. LPA provided and discussed LIC311A and the forms that must be in each child's and each staff member's file. INDOOR ACTIVITY SPACE: There is one toddler classroom (red) and two preschool classrooms (blue and green) located in the facility. There is a first aid kit located in each classroom and medications will be stored with the first aid kits and accordingly to each individual child and is inaccessible to children. LPA observed cleaning disinfectants are appropriately stored and inaccessible to children in each classroom. LPA observed a sufficient number of napping cots and mats and blankets, pillows, and sheets will be provided by the facility. Applicant stated there are no poisons or firearms on the premises. Applicant stated drinking water will be provided via filtered pitcher water and refrigerators. LPA observed a functional carbon monoxide detector in each classroom. Applicant stated they will use a digital sign-in/sign-out system and paper sign in/out system as a backup. There is a food preparation area located in the green preschool classroom and is inaccessible to children. There is a staff restroom located in the blue preschool classroom and in the main administrative office area. Report continues on page 2 Page 2 There are storage spaces in each classroom and are inaccessible to children. There is a dedicated landline for the facility. The isolation area for sick children will be located in the main administrative office area. Staff was reminded to keep all areas clean and free of litter to ensure the health and safety all children, staff and visitors. LPA measured the toddler classroom (red). The classroom space contains a total of 616.235 square feet, which will not accommodate the applicant's request for 20 toddler children. LPA measured the preschool classrooms (blue and green). The blue classroom contains a total of 712.871 square feet and the green classroom contains a total of 737.283 square feet and will accommodate the applicants request for 40 preschool children. LPA observed a sufficient amount of equipment, toys, tables, chairs, napping mats, and cubbies in each classroom. OUTDOOR ACTIVITY SPACE There are two outdoor play areas located at the facility, one for toddlers and one for preschool children. LPA measured the preschool outdoor area which includes a total of 4,238.28 square feet and will accommodate the applicants request for 40 preschool children. LPA measured the toddler outdoor area which includes a total of 1,531.62 square feet and will accommodate a maximum of 20 toddler children at one time. There are no bodies of water located onsite and there is adequate shade provided by pop up tents and awnings in both outdoor play areas which will be utilized during inclement weather. There is a brown wooden fence that is at least five feet tall surrounding the toddler and preschool outdoor play area and a brown fence separating the each area from the other. There is a play structure in the preschool outdoor area that is approved for ages 3-5 as per applicant Angela Evans. LPA observed sufficient play equipment and resilient padding in the toddler and preschool play areas. Report continues on page 3 Page 3 This facility plans to provide Incidental Medical Services – IMS. For IMS information, see PIN 22-02-CCP. A Plan of Operation that includes IMS must be submitted to the Department. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice) or (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm LPA discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds. LPA discussed with applicant any changes that may occur regarding the director or an employee acting in the director's absence must be reported to the department within 10 working days. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platform. Report continues on page 4 Page 4 Assembly Bill (AB) 2370, /chapter 676, Statues of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-year after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP) Exit interview conducted and report was reviewed with Applicant Angela Evans, Administrative Assistant Shayna Lee, Director Mona Chivers, and Exective Director Stevie Lee.
