What the inspector wrote

On October 2, 2025, Licensing Program Analysts (LPAs) Van and Naves conducted an annual inspection, which included checking the interior and exterior of the facility and discussing the required childcare paperwork with the Licensee. LPAs met with the licensee, Teri Rushworth. The purpose of the inspection was explained, and the Licensee granted LPA entry to the home. Upon entering the house, LPAs observed that the children were engaged in free play. At the time of inspection, the licensee and her husband were supervising seven children, one of whom was the licensee\342\200\231s grandson. All adults living or working in the home have a criminal record clearance verified with the CDSS Guardian Background Check System. According to the licensee, the childcare areas previously approved remain the same, with the same off-limits and childcare areas. The childcare areas are the playroom (converted garage), bathroom #1, master bedroom #1, Living Room, and the entire backyard. Off-limits areas are bedroom #2 and #3, bathroom #2 and #3, and storage space in the backyard. LPAs reminded the licensee to ensure all off-limit doors and entrances are barricaded or have child-proof doorknobs. The licensee acknowledged this and stated she will install child-proof doors on all doors and will send pictures to LPAs. The operation hours are Tuesday through Thursday from 8:00 a.m. to 4:00 p.m. The Licensee has valid childcare liability insurance on file. Continued on page 2. page 2. LPAs toured and inspected the Licensee's home to identify any potential health and safety risks. The facility is well-maintained, with adequate temperature control and ventilation. The Licensee offers a diverse range of toys and equipment suitable for children of various ages. All cleaning products, harmful substances, and other chemicals are stored out of reach of the children. The facility is equipped with a fully charged fire extinguisher, smoke alarms, carbon monoxide detectors, and a functioning phone. LPAs noticed several playpens in the home. The playpens were observed empty of toys, blankets, or pillows. The mattress fit properly, and the sheet was securely in place. The 15-minute sleep logs for infants were reviewed, and it was confirmed that the licensee had recorded the logs, including the date and the 15-minute increment check. LPA reminded the licensee that sleeping sacks are not allowed. LPAs also reminded the licensee to keep at least 30 days of logs available for review during the annual inspection and to retain records for a period of three years. The facility has a fully stocked first aid kit for children. No pool was observed, but the licensee has a hot tub in the backyard. The hot tub was observed with a hard cover. The backyard is completely fenced and well-stocked with a variety of outdoor toys and equipment in good condition. LPAs reviewed the children's records. The Licensee keeps thorough records for all children in their care, including vaccination records and Parental Rights forms. LPAs noted that each child has a complete emergency information card with the child's full name, phone number, and the contact information of a parent or authorized person who can be called in an emergency. The Licensee's Pediatric First Aid/CPR certification is valid until 2026. The Licensee has maintained her immunizations and completed the Mandated Reporter Training, as required by AB 1207. Continued on page 3. page 3. The most recent fire & earthquake drill was completed on August 5, 2025, and properly documented. All required licensing postings, including the facility license, parents' notification, and earthquake preparedness checklist, were properly posted. Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated. Incidental Medical Services (IMS) policy was discussed. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. LPAs also informed the Licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Licensee was informed that as of September 1, 2016, a person may not be employed or volunteer at a childcare facility unless he or she has been immunized against influenza, pertussis, and measles or qualifies for an exemption pursuant to Health and Safety code 1596.7995 and 1597.662. LPA reviewed AB 1207 with the Licensees. As of January 1, 2018, all staff must complete Mandated Reporter Training every two years. The training can be obtained online at www.mandatedreporterca.com . Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to childcare providers and Resource and Referral Agencies (R&Rs) throughout California. During the exit interview, the TERI RUSHWORTH, confirmed that there are no Registered Sex Offenders living in the facility, and LPA completed the RSO profile in FAS. A notice of site visit was given and must remain posted for 30 days. An exit interview was conducted, and the report was reviewed with the licensee, Teri Rushworth.
