An unannounced Case Management visit was conducted at the facility by Licensing Program Analyst (LPA) Robert Maciel to investigate circumstances involving two self-reported Unusual Incident Reports received by the department on 11/17/25 regarding a child (C1) allegedly being pinched by a staff member on the arm. LPA met with Facility Representative Kara Hess (S2). During today's visit, LPA observed 7 children in care. LPA conducted interviews with staff (S1, S2, S3, and S4) and children (C1 and C2). Further investigation and a follow up visit is warranted. No deficiencies were observed at this time. Appeal rights were provided. Report was reviewed and read to the Facility Representative Kara Hess. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
