A Case Management visit was conducted at the facility by Licensing Program Analysts (LPAs) Robert Maciel and Dianne Morrison in response to a self-reported Unusual Incident Report received by the department on 12/8/25 regarding a child (C1) who was left on the playground during transition inside. LPAs met with Facility Representative Tanyjasu Simms (D1). LPAs interviewed D1 who stated that on 12/8/25, a child (C1) was left outside on the playground for less than a minute. LPAs reviewed camera footage which show that C1 was outside by himself from 11:20 AM to 11:21 AM. Further investigation and a follow up visit is warranted. No deficiencies were observed during today's visit. Appeal rights were provided. Report was reviewed and Facility Representative Tanyjasu Simms. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
