What the inspector wrote

A Case Management visit was conducted at the facility by Licensing Program Analysts (LPAs) Robert Maciel and Dianne Morrison in response to a self-reported Unusual Incident Report received by the department on 11/21/25 regarding a child (C1) who was left on the playground during transition inside. LPAs met with Site Supervisor Kara Hess (D1). According to the incident report and interviews conducted, at around 11:45 AM on November 21, 2025, facility staff were transitioning children from the outdoor playground to the classroom. Ten of the children outside were taken inside by staff 2 (S2) to get ready for lunch and the remaining children stayed outside with staff 1 (S1) to continue participating in an activity. S1 stated that when transitioning inside, C1 was not visible from the door and was under the playground slide. Later, a teacher from the Yokayo State Preschool (# 233008083 ) observed C1 on the playground alone and notified facility staff, who went outside to bring her in. LPAs reviewed camera footage of the outdoor play area from 11/21/25 which revealed that C1 was alone on the playground from 11:46 AM to 11:52 AM. D1 stated that all staff are taking training on zoning for field of vision while outside and active supervision and that on 12/3/25, the North Coast Opportunity Education Specialist came to the site to review training on transitions, supervision and staff communication. The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 809D. Appeal rights were provided. Report was reviewed and Site Supervisor Kara Hess. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
