An unannounced case management visit was made to the facility by Licensing Program Analyst (LPA) Robert Maciel for the purpose of issuing a citation that was intended to be issued during a case management visit on 12/19/25 but was not by mistake. LPA met with Site Supervisor Tanyjasu Simms (D1). The citation delivered is in regards to an unusual incident report received by the department on 12/10/25 in which a child was accidentally left outside of the classroom during transition from the playground. LPA interviewed staff and reviewed camera footage on 12/19/25 which showed that a child (C1) was left outside on the ramp in front of the classroom door without supervision from 10:38 AM to 10:40 AM. D1 stated that all staff are taking training on active supervision and that the transition policy will be updated to include recounting children after transition inside the room as well as outside the door and on the playground before coming inside. The following violation of the California Code of Regulations, Title 22; Division 12, was observed: see LIC 809D. Appeal rights were provided. Report was reviewed and Site Supervisor Tanyjasu Simms . A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
