What Happened

Incident follow-up: tree with large crack fell on play structure on 4/9/25 at 11:20 AM. All staff and children were inside. Tree removed 4/10/25; demolished play structure debris remained on site pending removal by 4/18/25. Director failed to report incident within 24 hours as required.

Observations

  • Child noticed large crack in tree trunk at 9:30 AM on 4/9/25
  • Staff kept children away from tree and called arborist
  • Tree fell at 11:20 AM destroying play structure while all were indoors
  • Arborist assessed damage at 2:30 PM
  • Tree removed 4/10/25 by tree service company
  • Play structure debris present during 4/16 inspection, scheduled removal by 4/18/25
  • Director stated incident not reported because no children were harmed
