What the inspector wrote

Licensing Program Analyst (LPA) Robert Maciel conducted an unannounced complaint visit, and met with Director Kristi Mccullough. It was alleged that a staff member restrained a child in care, specifically that on 11/16/23 and 2/14/24, C1 was restrained and prevented from leaving the bathroom. During today’s visit, the facility was toured, records were reviewed. Interviews with staff and adults conducted from 2/23/24 to 3/8/24 corroborate the allegation. Based on the evidence obtained, the preponderance of evidence standard has been met; therefore, the above allegation is found to be substantiated. California Code of Regulations, Title 22, is being cited on the attached LIC 9099D. Appeal rights were provided. Exit interview conducted, and report was reviewed with the Director Kristi Mccullough. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

This poses a potential health, safety or personal rights risk to persons in care.
