The licensee shall ensure that each child is accorded the following personal rights:Not to be locked in any room, building or center premises by day or night.This requirement is not met as evidenced by: Based on interviews and record review, C1 was restrained in the preschool bathroom on 11/16/23 by A9 and on 2/14/24 by A8. This poses a potential health, safety and personal rights risk to children in care.

In addition, a written report…shall be submitted to the Department within seven days...(1)Events reported shall include the following: (C)Any unusual incident or child absence that threatens the physical or emotional health or safety of any child.This requirement was not met as evidenced by: Based on interview and record review staff who directly observed or later became aware of the incidents did not report it to the Department and did not submit a written incident report as required. This poses a potential health, safety or personal rights risk to persons in care.
