What Happened
Follow-up case management visit regarding incident where child was unsupervised for approximately 3 minutes when they entered unlocked classroom on 3/10/25. Facility implemented corrective measures including relocating office desk for better visibility and locking playground door when children outside.
Observations
- Incident occurred 3/10/25: Child (C1) ran inside unlocked classroom without staff seeing them
- Director in office did not observe child until they knocked on door
- Child was unsupervised for approximately 3 minutes
- Office desk moved into main room for better visibility
- Playground door now locked when children outside - only staff can open
