What the inspector wrote

This is an amended report originally issued on 03/28/2025. Licensing Program Analyst (LPA) Robert Maciel made a case management visit to the facility for the purpose of following up on an Unusual Incident Report that was self-reported to the department on 3/11/25 and met with Director Lucia Vasquez. On 3/10/25 at around 11:00 AM, while children were playing outside in the playground, a child (C1) ran inside the classroom without being seen by staff 1 (S1). The door to the classroom was closed but unlocked. Director Lucia Vasquez was in the office and did not see the child until they knocked on the door of the office. Director then took C1 back outside to the playground. During today's inspection, the facility was toured LPA spoke with staff. LPA observed that the office desk had been moved into the main room and Director stated that the door to the playground was locked whenever children were outside so that it could only be opened by staff. Director also stated that the child was alone for about 3 minutes. The following deficiencies were observed during today's visit (see LIC809D). Exit interview given and report was read with director Lucia Vasquez. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.
