What the inspector wrote

A Case Management visit was conducted at the facility by Licensing Program Analysts (LPAs) Robert Maciel-Kashima and Ivet Zamora Perez in response to two self-reported Unusual Incident Reports (UIRs) received by the department on 5/20/26 and 5/26/26. LPAs met with Director Lucia Vasquez (D1). The UIR submitted on 5/20/26 details an incident in which a parent alleged that a staff member (S1) inappropriately handled a child in care, specifically by grabbing a child by the shoulders too forcefully . LPAs interviewed children, staff, and adults which did not corroborate any evidence to support the allegation. The UIR submitted on 5/26/26 details an incident on 2/2/26 in which a child was released to the playground by S1 where no staff were present. LPAs reviewed facility camera footage which showed that the child was alone for two and a half minutes before another staff, S2, approached the playground. Director stated she was not present at the facility during the time of the incident and was not made aware of the incident later. The following deficiencies were observed during today's visit (see LIC809-D). Appeal rights were provided. Report was reviewed and Director Lucia Vasquez. A notice of site visit was given and must remain posted for 30 days. Failure to do so shall result in an immediate civil penalty of $100.

    Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event.This regulation was not met as evidenced by Based on written UIR and Director statement, an unusual incident occured on 2/2/26 which was not reported to the department until 5/21/26 which poses a risk to the health, safety, and personal rights of children in care.

    No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This regulation was not met as evidenced by Based on camera footage and Director statement, a child was released to the playground by S1 where no staff were present and was alone without supervision for two and a half minutes which poses a potenetial risk to the health, safety, or personal rights of children in care.
