Follow-up on corrections·October 23, 2025

A case management deficiencies visit was made to the facility today by Licensing Program Analysts (LPAs) Robert Maciel and Jessica Gaumann who met with Licensee (LS) Ericka Macias Ballesteros. As part of a complaint investigation, LS showed LPA Maciel camera footage from the Ring camera in the playroom on the day of 7/30/25. A clip taken at 1:23 PM shows an infant in a playpen with a blanket. The infant is awake. In a different clip taken at 2:07 PM, LS is waking up the children from nap and turns the light on in the playroom showing the infant sitting up in the playpen with the same blanket. LS stated that the infant pulled the blanket into the play pen and has been given blankets by other children in the nap room while they are napping. Licensee was cited for the same deficiency on 3/28/25. Exit interview conducted and report was reviewed with the Licensee, Ericka Macias Ballesteros. The following violation(s) of California Code of Regulations, Title 22; Division 12, were observed during today’s visit, see LIC809-D. Appeal rights were provided. Civil penalties are assessed on form LIC421FC for repeat violations within 12 months, an immediate $250 is being assessed today.
