What the inspector wrote

On 01/29/2026, Licensing Program Analyst (LPA) Pa Kou Vue conducted an unannounced Case Management – Incident Inspection at the facility. LPA met with Reporting Party (RP) - Site Supervisor Jacqueline Mendoza Quezada and explained the purpose of the inspection. LPA toured the facility and took a census. On 11/17/2025, Site Supervisor Jacqueline Mendoza Quezada called Childcare Licensing (CCL) to report an incident regarding C1 due to a potential personal rights violation that occurred in classroom #1. On 12/11/2025, LPA conducted a follow-up telephone call with RP. LPA obtained records and interviews from RP via email. RP has been in active communications with LPA throughout the investigation. On today’s date, LPA conducted an interview with Site Supervisor Jacqueline Mendoza Quezada and LPA was accompanied on a tour of classroom #1 and the gym. In addition, LPA confirmed the teacher – child ratio for the classroom #1 on 11/17/2025 was 16 children with 3 staff members – S1, S2 and S3. At this time, it cannot be determined if the incident in reference is an isolated incident due to a pending a internal investigation. Site Supervisor stated she will be in active communications with LPA and will ensure compliance. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited. Exit interview conducted and report was reviewed Site Supervisor Jacqueline Mendoza. Appeal rights were provided. A notice of site visit was given and must remain posted for 30 days.
