What the inspector wrote

On 05/26/26, Licensing Program Analyst (LPA) Yesenia Fierro conducted an unannounced case management inspection. LPA Fierro met with site supervisor Jacqueline Mendoza Quezada. The purpose of today’s inspection was to conduct a case management inspection and to follow up with the facility regarding a lead exceedance referred to as an Action Level Exceedance (ALE) of over 5.5 parts per billion (ppb) in the water at the facility. LPA conducted a tour of the facility inside and out and a census was taken. Fresno Regional Office (FRO) received notification that the facility had a high lead exceedance from water lead sampling tests collected on 11/15/22. During a Case Management inspection on 12/14/22, it was found that Outlet 240406429 H was completely removed. Per site supervisor Jacqueline Mendoza Quezada and head of maintenance Nick, the facility has since remediated the exceedance by removing outlet 240406429 H. During today’s inspection LPA observed outlet 240406429 H has been completely removed. It should also be noted the outlet 240406429 H is located in a classroom that is not licensed for childcare. LPA Fierro received retesting results dated 06/06/23 showing that the lead testing results were below 5.5 parts per billion (ppb) for outlets 240406429 I, 240406429 J, 240406429 K, 240406429 L, 240406429 I30. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited during today's inspection. An exit interview was conducted with site supervisor Jacqueline Mendoza Quezada. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
