What the inspector wrote

On 12/12/2023, Licensing Program Analyst (LPA) Valerie Mireles met with Director See Lao for an unannounced case management inspection. A complete file review was conducted prior to today's inspection. LPAs toured the facility and a census was taken. LPA observed 17 preschool children supervised by three staff, including one fully qualified Preschool Teacher. The purpose of today's inspection was to address an unusual incident that took place at the facility on 11/03/2023. An Unusual Incident Report was submitted to the Fresno Community Care Licensing (CCL) Office regarding an incident, that occurred on 11/03/2023 at 9:55 a.m., involving a preschool child that fell while climbing the curved ladder of the outdoor play structure and fell, resulting in the child hitting their head on the metal rail and sustaining a small red bump on the back of their head. On 12/12/2023, LPA spoke with Staff #1, who was not present during the incident, but spoke to the parent of the child. According to Staff#1, the child has a pre-existing medical condition of persistent headaches unrelated to the incident and had already been receiving medical treatment for the headaches; therefore, the parent opted to monitor the child from home. Immediately after the incident, the child was taken to the school nurse and aide was rendered. The child\342\200\231s parent was notified, and the child was picked up. The child was not taken to doctor for follow-up regarding the incident and returned to preschool the following day. On 12/12/2023, LPA observed the outdoor play structure where the incident took place and photos were taken. The outdoor play structure has a curved climbing ladder, which is where the incident took place. The playground structure has rubber cushioning at the base of the structure to absorb falls. At the time of the incident there were three staff present and approximately 20 preschool children present during outdoor play; therefore, adequate supervision was in place. Based on the information obtained, LPA determined staff handled the incident correctly and reporting requirements were met. After interviewing staff, touring the facility and reviewing facility records, LPA determined facility staff took appropriate measures to address the child\342\200\231s injury, following proper policies and procedures and no regulations were violated. Per Staff#2, the faiclity will take further measures by having staff talk to students about being safe and reiterate the rules and safety guidelines to students. Continued to LIC809-C. Per the California Code of Regulations, Title 22, Division 12, Chapter 1, no deficiency was cited during today\342\200\231s inspection. Exit interview conducted with the Director, See Lao. This report shall be made available to the public upon request. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
