What the inspector wrote

On 1/18/2022, Licensing Program Analyst (LPA) Jeovanna Yanez conducted an unannounced case management inspection. LPA toured the facility and a census was taken. LPA met with Site Supervisor, Kathleen Thai. The purpose today's inspection was to discuss an incident that occurred on 11/10/21, in which Child #1 sustained two broken bones on their left arm. This incident was reported to the Fresno Community Care Licensing Office on 11/10/21. During today's inspection, LPA interviewed Site Supervisor, and Staff #1 whom were associated with the incident. Information obtained revealed that Child #1 was playing with a large group of children in the outdoor playground, running up and down the grass area. As Child #1 was turning themselves to run the opposite way, child #1 crossed one foot in front of the other and tripped on themselves. Child #1 fell to the ground and Staff #1 observed the incident and immediately began to assist Child #1. Staff #1 observed swelling on the child's arm and called for Site Supervisor for further assistance. Site supervisor then called Child #1 parents, who took the child to receive medical care and received a diagnosis of two broken bones. Child #1 has returned to care and is no longer wearing a cast. During today's inspection, LPA inspected the outdoor area where the incident occurred and did not observe any safety hazards or areas of concern. LPA discussed supervision at the time of the incident and it was determined that appropriate supervision was in place. This appears to be an isolated incident and staff took appropriate measures to address the child’s injury, following appropriate policies, regulations, and reporting requirements. Per Title 22, Division 12, Chapter 1, of the California Code of Regulations, no deficiencies are cited . Exit interview conducted and report was reviewed with Site Supervisor, Kathleen Thai. This report shall be made available to the public upon request. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
