What the inspector wrote

Licensing Program Analysts (LPAs) Kimberly Vass and Aurelio Mendoza conducted an unannounced Case Management \342\200\223 Incident inspection and met with Master Teacher Yesenia Sicairos. The facility self-reported an incident that occurred on 02/12/2026. Based on staff interviews and record review, the event occurred between approximately 11:30 a.m. and 11:32 a.m., immediately after children returned from the playground. During the transition, staff identified one child unaccounted for and immediately located the child in the playground, which remained within the classroom\342\200\231s line of sight. LPAs confirmed the child was unsupervised for up to two minutes during the transition period. During the playground walkthrough, LPAs observed the area to be fully encircled, secure, and in direct line of sight of the classroom. The fencing was found to be healthy, intact, and sound, mitigating risks of a child exiting the facility. LPAs consulted with Master Teacher Sicairos and reinforced the requirement for continuous visual supervision during all transitions. Documentation was provided verifying that all staff completed Active Supervision & Safety Training on 02/12/2026. Record review showed no pattern of similar incidents, and the incident was determined to be isolated, with staff responding appropriately and consistent with policies, regulations, and reporting requirements. An exit interview was conducted with Master Teacher Sicairos, and appeal rights were provided. Per Title 22, Division 12 of the California Code of Regulations, no deficiencies are cited. This report shall be made available to the public upon request. The LIC 9213 Notice of Site Visit was issued and must remain posted for 30 days.
