What the inspector wrote

On 10/21/2025, Licensing Program Analyst (LPA) Ka Vang conducted an unannounced case management inspection. LPA toured the facility, and a census was taken. LPA met with Andrea Ceja and Stephanie Segovia, Site Supervisors. Today\342\200\231s inspection was to discuss an unusual incident that occurred on 10/21/2025, in which Child #1 (C1) sustained an injury on her left eyebrow area. This incident was reported to the Fresno Community Care Licensing Office on 10/21/2025 by Site Supervisor Andrea Ceja. Per interviews, on 10/20/2025, there were two staff members (Staff #1 (S1) and Staff #2 (S2) supervised 4 infants in the Infant Classroom. As S2 was changing another infant's diaper, S1 was next to the wall cabinet filling out documentation while both staff members have direct vision of the infants. S1 observed that C1 was walking toward the infant cubbies and started to pulled her personal items from the cubbies, S1 informed C1 that C1 can not take her personal clothes out. S1 observed C1 was little bit upset but C1 did not take her personal items out from her cubby. As C1 turned around from the cubbies, C1 lost balance and fell forward and hit C1's left eyebrow on the wooded wall cabinet next to the cubbies. S1 immediately picked up C1 and assisted C1. S1 observed that there was a small cut and bruise on C1\342\200\231s left eye brown, S1 contact site supervisor while applying first aid on C1\342\200\231s wounded area. Site Supervisor went to the Infant Classroom and observed that infant was crying, Site Supervisor observed that there was no heaving bleeding or large open wound, Site Supervisor took C1 to her office and immediately contacted C1\342\200\231s authorized representative (AR) and informed C1\342\200\231s AR of the incident. (Continued on LIC809-C). LPA reviewed pertinent documentation at the facility. LPA verified teacher-child ratios were being followed during time of incident. Based on the information obtained, this appears to be an isolated incident, and staff members took appropriate measures to address the child's injury, following proper policies and procedures and no regulations were violated. Per Title 22, Division 12, Chapter 1 of the California Code of Regulations, no deficiency is being cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with Site Supervisor, Stephanie Segovia. Stephanie was provided with appeal rights. LIC 9213 Notice of Site Visit is provided and required to be posted for 30 days.
