What the inspector wrote

On 09/08/25, Licensing Program Analyst (LPA) Martha De Haro conducted an unannounced case management inspection. LPA toured the facility, and a census was taken. LPA met with Site Supervisor Araceli Rendon. The purpose of today's inspection was to discuss an unusual incident, which occurred on 08/26/25 in which a child was left unsupervised in the outdoor play area. During the case management inspection, LPA interviewed the Site Supervisor as well as the preschool teachers and received and gathered additional documentation. After conducting the case management inspection and interviewing staff, it was found that child #1 was left unsupervised in the outdoor play area for approximately seven (7) to eight (8) seconds. Per staff interviews, there were ten (10) children present on the day of the incident along with two (2) staff members. During the time when the children were about to transition from playing outdoors to walking back into the classroom, staff #1 stood at the front of the line and had the children line up near the entrance to the classroom. Staff #1 counted the children and verified that there were ten (10) children in line. As staff #1 and the children began to enter the classroom, staff #2 was at the back of the line. Staff #2 was in charge of ensuring that all of the children walked inside of the classroom while she removed the red safety bag from a hook near the door and brought in the radio and children\342\200\231s roster. Staff #2 did not notice that the last child in the line (child #1) had left the line. Child #1 had moved out of staff #2\342\200\231s line of sight while staff #2\342\200\231s back was turned as she picked up the items that she needed to bring inside. Upon entering the classroom, one of the children pointed out to staff #2 that child #1 was standing on the other side of the closed door, looking inside through the glass in the door. Child #1 was brought back in to the classroom. Per the Site Supervisor, the parents were notified of the incident right away and child #1 resumed normal activities on that day. (Continued on LIC 809-C) Per Site Supervisor Ms. Rendon, she held a training with preschool staff on 09/03/25 in which they developed an action plan in order to ensure that children are not left without supervision and reviewed active supervision strategies. They also made changes to their protocol for when bringing in children from outdoors in order to ensure that this does not happen again. The Teacher (staff #1) will now be in charge of bringing in the red safety bag and will continue to stand in front of the line, counting the children out loud as they walk inside. The assistant (staff #2) will ensure that she picks up the radio and children\342\200\231s roster while the children are still in line, will also count the children, and will make a thorough sweep of the playground before walking into the classroom behind the line of children. Both staff members will also recount the children once back inside of the classroom. Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, the following deficiency is being cited during today\342\200\231s inspection (See LIC 809-D). A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Site Supervisor Araceli Rendon and a copy of the report and appeal rights were given and discussed.
