What the inspector wrote

On 12/02/24, Licensing Program Analyst (LPA) Martha De Haro conducted an unannounced case management inspection. LPA toured the facility, and a census was taken. LPA met with Site Supervisor Melanie Flores. The purpose of today's inspection was to discuss an unusual incident, which occurred on 11/08/24 in which child #1 was reported to have been injured at the facility. During the case management inspection, LPA conducted facility observations, interviewed the Site Supervisor as well as staff, and reviewed children and staff files. After conducting the case management inspection and interviewing staff, it was found that each of the staff members interviewed denied observing anything out of the ordinary occurring with child #1. Staff reported that child #1 was having a normal day on 11/08/24, he did not appear to be in any pain while at the facility, nor did he ever become injured while at the facility. The Site Supervisor reported that in her direct interactions with child #1, she picked him up and held him on several occasions throughout the day and the child never expressed discomfort or demonstrated that he was in pain due to an injury. The toddler teacher denied seeing child #1 becoming injured nor did she observe him to be in any pain during her interactions with him. The toddler teacher denied that she did anything to injure the child while he was in her care. Per Chapter 1, Division 12, Title 22 of the California Code of Regulations, no deficiencies are being cited during today’s inspection. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted with Site Supervisor Melanie Flores and a copy of the report and appeal rights were given and discussed.
