Based on interview and record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.During employee records review, Licensing Program Analyst (LPA) observed that Assistant #1 and Assistant #2 were missing proof of immunizations (Measles, Pertussis, and flu). Assistant #1 was also missing proof of Tuberculosis testing.
