What Happened

Unannounced annual inspection of family childcare home. Two cribs with safety concerns identified: one crib next to front door with TV mounted directly above, second crib with farmhouse clock above. Outdoor climbing structure tilted and not sturdy. Licensee and assistant CPR/Mandated Reporter trainings expiring soon.

  • Climbing structure tilted and not sturdy (moves when tugged against)
  • Shade coverings for climbing structure torn
  • Licensee has had the tilted structure with her own children
  • Licensee's Mandated Reporter Training expires 04/29/2025
  • Assistant's Mandated Reporter Training expired (last completed 09/22/22)
  • Licensee's pediatric CPR/First Aid expires 04/15/2025
  • Assistant's pediatric CPR/First Aid expires 09/2026
  • All employees/volunteers have immunization records (influenza, pertussis, measles)

Based on LPA observations and Licensee interview, the licensee did not comply with the section cited above in LPA observed a play structure consisting of 3 swings attached to a climbing structure and a slide in accessible backyard. The climbing structure is tilted and is not sturdy and moves when tugged against which poses/posed a potential health, safety or personal rights risk to persons in care.
