An unannounced case management inspection was conducted today on 05/28/26 1:24pm by Licensing Program Analyst (LPA), Sydney Sims and Erica Laird. LPA met with facility representative Vickie Northrup. In response to an unusual incident report received by the department on 05/15/26 where on 05/14/26 the facility took a field trip to the local elementary school and child C1 was handled roughly by an elementary school staff. The facility representative (S3) was interviewed on 05/28/26 at 1:26pm and stated that on 05/14/26 during the time of the incident S3 observed witness (W1) grab child C1 roughly by the fingers and pulled C1's arm back. S3 stated that C1 was not alone with W1 and that staff S2 was with C1 at the time of the incident providing proper supervision. Two Staff (S1 – S2) were interviewed on 05/28/26 and S1 stated that S1 did not observe an interaction between C1 and W1 but that S1 was providing supervision for two other children. S2 stated that S2 was with C1 at the time of the incident but that S2 did not observe W1 handle C1 roughly at any point. S1 and S2 confirmed that constant supervision was being provided by the facility staff for C1. Parent (P1) was interviewed on 05/28/26 and stated that P1 was informed about the incident and that P1 went to the school and reviewed the camera footage of the incident. P1 stated that P1 did not observe C1 being mishandled. P1 stated that P1 observed in the video that facility staff S1 was providing supervision to child C1, and that P1 had no concerns about supervision. During today’s inspection, the facility was toured, interviews were conducted with staff and parents, and the children’s record were reviewed. Based on interviews and reports received by the department, it could not be determined that a regulation violation occurred. Exit interview conducted and report was reviewed with the facility representative Vickie Northrup. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
