What the inspector wrote

An unannounced case management inspection was conducted today at 10:45am by Licensing Program Analyst (LPA), Sydney Sims. LPA met with Director Luz Elena Cardenas. In response to an Unusual Incident Report received by the Department on 10/13/25, where child C1 sustained an injury to the head that resulted in a concussion and stitches on C1's forehead. The Director was interviewed on 10/24/25 at 11:15am and stated that on 10/7/25 at 10:50am child C1 was running in the classroom and tripped, and hit C1's head on a chair in the classroom resulting in a large cut to the left side of C1's head that required stitches. Director attempted to call C1's guardian but did not get an answer, and due to the nature of the injury the Director and S1 transported C1 to the local clinic. After receiving medical attention it was determined that C1 had a concussion. Director stated that staff S1 - S2 were present in the classroom at the time of the incident. Two staff (S1 -S2) were interviewed on 10/24/25 and stated that On 10/7/25 C1 was being disruptive of circle time so S1 took C1 over to a table do a puzzle, and while sitting at the table C1 got up and started to run then tripped on a chair while running and hit C1's head on another chair. C1 sustained a concussion and a large cut to the head that required stitches. S1 stated that S1 and the Director transported C1 to the local clinic because C1's guardian could not be reached. During today\342\200\231s inspection, the facility was toured and LPA Sims observed that the chairs in the classroom were age appropriate and had rounded corners and edges. LPA conducted a record review of sign in sheets and staff time sheets for 10/7/25 that showed the facility was in ratio. Based on interviews conducted with Director and Staff S1 - S2, record review, and facility tour it was determined that although child C1 did sustain an injury while in care it could not be determined that a regulation violation occurred. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the Director Luz Elena Cardenas. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
