What the inspector wrote

An unannounced case management inspection was conducted today at 1:10pm by Licensing Program Analyst (LPA), Sydney Sims. LPA met with Director Luz Elena Cardenas. In response to an Unusual Incident Report received by the Department on 9/24/25, where on 09/17/25 child C1 threw themselves on the ground and sprained C1's wrist. The Director was interviewed on 10/24/25 at 1:31pm and stated that on 09/17/25 at 10:40am child C1 was in the classroom with staff S1 and S2. Staff S1 was sitting down with children reading a book when C1 came over by the group and C1 threw them self on the ground next to the group and started crying. After C1 had stopped crying staff S2 noticed that C1 was not using one of C1's arms so S1 brought C1 to the Director who called C1's guardian to come pick up C1. Director stated that there was no other signs of injury like swelling or bruising. C1 received medical attention and it was determined C1 had a sprained wrist. Two staff (S1-S2) were interview on 10/24/25 and stated that S1 and S2 were in the classroom with C1 and other children, S1 was reading a book to the children and C1 came over to the circle and threw C1's self down on the ground near the group. S2 stated that S2 then took C1 away from the group to console and redirect C1, and gave C1 some activities to do but that C1 would only use one hand so S1 brought C1 to the Director. S1 stated that there was no other signs of injury like bruising or swelling. During today\342\200\231s inspection, the facility was toured and LPA observed 11 children in care. LPA Sims conducted a record review of C1's doctors note and admissions file, staff time sheets and children's attendance sheet. LPA observed that the facility was in ratio based off time sheets and attendance sheets. Based on interview interviews, and record review although, C1 did sustain an injury while in care it could not be determined that a regulation violation occurred. There were no deficiencies cited during today\342\200\231s inspection. Exit interview conducted and report was reviewed with the Director Luz Elena Cardenas. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
