Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA), Cortney Nelson, met with Office Manager, Brandy Griffin-Bagno, and explained purpose of today's visit. LPA was admitted into the facility by a staff member and was notified that Site Director, Heather Diaz, has left early for the day. LPA Nelson reviewed the facility's Parent Handbook and observed in the section relating to "Medication" the following statement, "Parent/legal guardian's AND physician's signatures are required on the diocesan medication form before medication of any kind can be administered by the director during school hours." A child in care provided EpiPen to the facility without the Permission to Administer Medication form completed, despite providing the form to parent via email in June 2023. LPA observed EpiPen was at the facility maintained in a ziplock bag without original packaging or completed paperwork. LPA additionally observed section in Parent Handbook "Nutrition-Lunches" and reviewed the following, "Please be mindful of any food allergies in your child's classroom. If there is a child with an EpiPen for nut products, his/her classroom becomes a nut free class." A child was in attendance in the Sea Otter classroom with an EpiPen for nut tree allergies and the classroom was not made nut free. The Site Director states that verbal conversations were made with the child's authorized representative regarding the classroom becoming nut free, however no changes were made. As a result of today's inspection, deficiencies have been cited, see LIC809-D. Exit interview conducted and report was reviewed with Office Manager, Brandy Griffin-Bagno. A NOTICE OF SITE VISIT WAS GIVEN AND MUST BE POSTED FOR 30 DAYS.
