What the inspector wrote

Licensing Program Analyst (LPA) Marilou Monico conducted a Case Management Inspection in response to licensee's request to change the capacity from 12 to 26 children and adding a classroom on the license. LPA met with Center Director, Jennifer Sebolino, and the purpose of the inspection was explained. The infant program operates Monday through Friday from 7:00 AM to 5:30 PM. LPA toured the facility both inside and out, as well as measured the space available for the children during today's visit. INDOOR MEASUREMENTS: INFANT ROOM: 25.833 x 19.083 = 492.971 minus 10.300 (encumbered space) = 482.671 TODDLER ROOM: 42.833 x 19.083 = 817.382 minus 158.011 (encumbered space) = 659.371 TOTAL PRESCHOOL INDOOR SPACE = 1,142.042 sq. ft. divided by 35 = 32 children The infant program has 4 sinks (60) and 1 toilet (15) available for the children. The sinks used by children only have cold running water. There is a staff bathroom adjacent to the kitchen. The isolation of sick children will be in the Office and isolation equipment will be provided. Sick children will use the staff bathroom if needed and will be accompanied by a staff member. LPA observed a diaper changing table within arm's reach of a sink in the Infant Room and in the Toddler Room. There are 34 chairs, 2 cribs, 9 tables, 26 cots, and 48 cubbies for the children. OUTDOOR MEASUREMENTS: INFANT/TODDLER PLAYGROUND: 104.667 x 73.000 = 7,640.691 minus 230.774 (encumbered space) = 7,409.917 TOTAL OUTDOOR SPACE = 7,409.917 sq. ft. divided by 75 = 98 children The playground is surrounded by appropriate fencing and the outdoor surfaces are safe. Shade is provided by trees, umbrellas, and canopies. The areas under and around the climbing structures are cushioned with resilient material (rubber mulch) that will absorb a fall. Drinking water is provided via pitcher and disposable/plastic cups . There were no bodies of water observed. Continuation on next page: Center Director, Jennifer Sebolino, was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated. An updated facility sketch, Emergency Disaster Plan (LIC 610), Designation of Facility Responsibility (LIC 308), and paperwork for the assistant director were received during the inspection. Exit interview was conducted and report was reviewed with Center Director, Jennifer Sebolino, and advised her that a license for 26 children will be granted upon receipt of the following: 1) Approved fire clearance. 2) Photos showing that the crib area is set up with cribs and gate has been installed. There were no deficiencies cited. A NOTICE OF SITE VISIT WAS GIVEN AND MUST REMAIN POSTED FOR 30 DAYS.
