What the inspector wrote

Licensing Program Analyst (LPA) Elizabeth Larios, conducted an unannounced case management visit today. LPA met with Coordinator, Nitaya Robinson & Site Supervisor, Ashley Reyes and discussed the nature of today's visit. LPA conducted a complaint investigation on May 20, 2025 and inspected classrooms. LPA observed disinfecting wipes in classroom 1 within reach of children. LPA observed peeling paint in walls of classrooms 1, 4, and 5B. LPA also observed a dead fly and other insects on the windowsill of classroom 8. Classroom 2 sink was leaking water on floor making it a slipping hazard. The following "Type B " deficiencies was cited during visit. See LIC 809-D for more details. Exit interview was conducted and report was reviewed with Coordinator, Nitaya Robinson & Site Supervisor, Ashley Reyes. Appeal Rights were given. A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.
