Inspector’s narrative

What the inspector wrote

Site Supervisor self-admitted that the child (C1) was left in the library(Room 16), without any teacher being present for about an hour and half as child (C1) was sleeping in the library(Room 16) during morning after child was dropped off at the facility, on May 12, 2025. Based on interviews, staff left a child(C1) unattended in library(Room 16) without any teacher being present on May 12, 2025 during morning time. Based on interviews, four(4) children(C2, C3, C4 &C5) were left unattended in the bathroom on May 30, 2025. Site supervisor stated that they are not aware of the time duration of four children(C2, C3, C4 &C5) were being left unsupervised in the bathroom. Therefore, the above allegation is SUBSTANTIATED, meaning the allegation is valid because the preponderance of the evidence standard has been met.
