What the inspector wrote

Licensing Program Analyst (LPA) Mandeep Kaur, conducted an unannounced case management inspection today. LPA met with Director, Nitaya Robinson & Site Supervisor, Ashley Reyes and discussed the nature of today's inspection. LPA interviewed Director, Site Supervisor and Staff (S1&S2&S3) during complaint investigation on June 26, 2025. During the interviews with staff, site supervisor and Director, LPA concludes that the department was not notified of the the incidents occurred on May 12, 2025 where a child (C1) was left unattended for an hour and half in the library (Room 16) without any teacher being present and second incident occurred on May 30, 2025, four(4) children(C2, C3, C4 &C5) were left unattended in the bathroom without any visual observations. Site supervisor stated that the children (C1, C2, C3 C4 & C5) were safe and they thought that they would be able to take care of the situation internally. During today's inspection, one "Type B" deficiency is issued on attached- LIC 809-D. Appeal rights provided Exit interview was conducted and report was reviewed with Director, Nitaya Robinson and Site Supervisor, Ashley Reyes. A NOTICE OF SITE VISIT WAS ISSUED AND MUST REMAIN POSTED FOR 30 CONSECUTIVE DAYS.

    Responsibility for Providing Care and Supervision: (a)The licensee shall provide care and supervision as necessary to meet the children's needs. (1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.This requirement was not met as evidenced by: Site Supervisor self-admitted that the child (C1) was left in the library, without any teacher being present for about an hour and half as child (C1) was sleeping in the library(Room 16) during morning after child was dropped off at the facility, on May 12, 2025 and based on interviews, four(4) children(C2, C3, C4 &C5) were left unattended in the bathroom on May 30, 2025, which poses an immediate risk to the health, safety and personal rights of children in care.
