What the inspector wrote

Licensing Program Analysts (LPAs) Marilou Monico and Dung Mac, conducted an announced pre-licensing inspection to the Facility today. LPAs met with Shannan Watkins, Applicant Representative, and Tiffany Allen, Site Director, and explained the nature of today's inspection. The Applicant has applied for a preschool (#274416996) and infant (#274416997) on the same premises. The Preschool program will operate Monday through Friday from 07:00AM to 05:30PM , serving Preschool children ages 24 months to entry into first grade . The Preschool program will be utilizing Day Care Room and the Infant program will be using the Infant Room and Toddler Room. LPAs toured and measured the indoor and outdoor areas of the Facility during today's inspection. INDOOR MEASUREMENTS ARE AS FOLLOWS: Day Care Room = 35.75 x 26.58 = 950.235 - 91.739 (encumbered space) = 858.496 TOTAL INDOOR SPACE = 858.496 sq. ft. divided 35 sq. ft. = 24 children There are 3 sinks (45) and 2 toilets (30) available for the preschool children. There are 4 tables, 30 cots, 25 chairs, and 24 cubbies. LPAs observed containers with tight-fitting lids for the disposal of solid waste. The Facility has central heating/air conditioning. The Day Care Room has appropriate lighting. Drinking water is available for preschool indoor via pitchers and plastic cups. Continuation on page #2 LPAs observed functioning smoke/carbon monoxide detectors at the facility. Cleaning supplies will be stored on high shelves in the classroom. First aid supplies are stored in the classroom inaccessible to the children. Medications will be stored in a lock box installed on the wall located by the kitchen area out of children's reach. There is adequate equipment, supplies, and toys for the preschool children. The facility has two separate staff bathrooms. The staff bathroom located on the right corner will be utilized by sick children if necessary. Sick children will be isolated in the director's office and isolation equipment will be provided. The second floor of the facility is off limits to children. Shannan states that the second floor will be used for staff training purposes and college courses classroom after 6:00 PM. The facility will be providing breakfast, lunch and afternoon snacks. There is a kitchen on-site and the food preparation area is functional and has hot and cold running water, refrigerator, sink, dishwasher, stove, oven, and storage space for food. Cleaning and sanitation of dishes and utensils will be done via dishwasher. Plastic utensils and plates will be utilized by the Facility. Transportation will not be provided. LPAs did not observe any bodies of water. Shannan states that there are no weapons on the premises. OUTDOOR MEASUREMENTS ARE AS FOLLOWS: Preschool Yard = 83.000 x 32.67 = 2,711.61 TOTAL OUTDOOR SPACE = 2.711 sq. ft. divided by 35 sq. ft. = 36 children Preschool Yard is exclusively for Preschool program and is surrounded by appropriate fencing. Shade is provided by canopy, trees, and building overhang. There are adequate supplies and toys for children outdoor. Drinking water will be provided outdoor via drinking fountains. Continued on Page #3 Shannan Watkins, Applicant Representative, and Tiffany Allen, Site Director, have obtained a Criminal Records Clearance and Child Abuse Index Checks. Site Director, Tiffany, has met the education and experience qualification requirements and her Pediatric First Aid/CPR Certifications expire July 23, 2023. Tiffany has proof of completion of Preventative Health Practices and immunization in Measles and Pertussis and Influenza on file. LPAs reminded Tiffany of the applicable civil penalties for those adults who have not received fingerprint clearances, are not associated to the facility, and who come in contact with or provide care and supervision to the children. For an initial violation, civil penalty amounts to $100.00 per person per day up to $500.00 per person. For a subsequent violation within a 12-month period, civil penalty amounts to $100.00 per person up to $3000.00 per person. This facility plans to provide Incidental Medical Services \342\200\223 IMS. A Plan of Operation that includes IMS has been submitted to the Department. Updated approved fire clearance was received during the inspection. LPAs conducted an exit Interview and advised Tiffany that a license for 24 preschool children will be granted pending Licensing Management review and approval.
